How to get a health card?

To utilize the service at any of the Hamad Medical Corporation’s healthcare facilities or hospitals, you can apply for health card. Consultations will cost extra but will be subsidized for those who hold a valid health card. Health card holders also have their prescriptions filled at a pharmacy run by the government at subsidized rates.

Health card contains ID number, nationality, photo and date of issue. It looks like a driving license. All the residents of Qatar are eligible for this health card. 

You just need to visit Primary Health Care Center and fill the appropriate form to get the health card. 

Required Doccuments:

*Qatari Adults: Passport copy or ID, a passport size photo and a fee of QR50. 

*Qatari Children: Birth certificate, passport size photo, vaccination card and a fee of QR50.

*GCC National Adult Residants: Passport copy, ID with Qatar personal identity number,  a passport size photo and a fee of QR50. 

*GCC National Child Residants: Birth certificate, passport size photo, vaccination card and a fee of QR50.

*Adult Residants: Valid Qatar ID or passport with valid residence permit, a passport-size photo and a fee of QR100.

 

*Children: Valid Qatar ID or passport with valid residence permit, vaccination card for those who have born in Qatar and a passport-size photo and a fee of QR100.

*Domestic Staff: passport with valid residence permit, a passport-size photo and a fee of QR50 and original Qatar ID of sponsor.

Renewal of health Card:

*A simple process which can be done at all the Hamad Medical Corporation Hospitals. 

*It can be done through online by visiting government's web portal (http://portal.www.gov.qa/wps/portal)

*If you lose your valid healht card, you need to pay QR50 to get a replacement (If you are a Qatari)  and you need to pay QR100 (If you are a non-Qatari). And for expired healthcard, Qatari nationals need to pay QR100 and non Qataris need to pay QR200.
